SENTARA REGIONAL RESOURCE POOL LOCATED IN HAMPTON ROADS IS HIRING AN INPATIENT CASE MANAGER, RN -DAYS/FLEXI FOR THE RESOURCE POOL.

RN Integrated Care Manager at Sentara Healthcare

Work Requirements:
--- Experience: 18 months of recent experience.
--- Facility supported: Members of the team are required to work assigned shifts in all units within the case management specialty. Required to work in 4 hospitals in the Southside or Western Tidewater regions. There are three region options.
--- Operational hours = 8:00 AM - 4:30 PM 7 days per week
--- Flexi - Hours: The team member must enter at least 48 hours of prescheduled time per calendar month in API. Of these prescheduled hours, 16 hours are to include availability to be scheduled on a weekend.

Hospital Region Option:
--- Southside Locations only (SNGH, SLH, SPAH, SVBGH)
--- Peninsula Locations only (SOH, SCH, SWRMC, SAMC)
--- Western Tidewater (2 Southside and 2 Peninsula) SNGH, SLH, SPAH, SVBGH SOH, SCH, SWRMC, SAMC

--- Requirements
(Prescheduled hours: Defined as clinical hours scheduled 4 weeks in advance of the current date.) Education and orientation hours will not count toward prescheduled or work hours requirements.
--- Weekend Definition: Any shift from 7P Friday to 0700 Monday
--- Off-shifts are defined as:
o Any time after 1500
--- 8- and 12-hour increments: 7A (the 4 hours from will count as 4 hours off shift worked

As an Inpatient Care Manager, responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, psychological, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient. Collaborates with patients, families, healthcare team members, and community organizations, and uses strong advocacy skills to identify needs and resources in the hospital and community.

Develop and evaluates effective transition plans taking into consideration the client's diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care. Functions in one of the following practice settings: Hospitals Only

Job Summary

Responsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs. Functions in one of the following practice settings: Acute Care, Service Lines, Ambulatory/Community-based, Home Health, and Long Term Care.

BLS required within 90 days of hire and maintained thereafter (if in clinical setting)

In Behavioral Health - De-escalation and physical intervention training within 15 days of hire.

For Integrated Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC or CGMT-BC). For other service lines, certification based on specialty area required within one year of eligibility.

3 years Case Management experience preferred.

For those in Behavioral Health - Certification in de-escalation training within 15 days of hire and annually.

PACE specific incumbents for this position require a minimum of one year of experience working with the frail or elderly population.

Qualifications:

N-2YR - RN-Associate's Degree, N-3YR - RN-Diploma (Non-degree), N-4YR - RN-Bachelor's Level Degree, N-6YR - RN-Master's Level Degree

Registered Nurse (RN) Single State - Nursing License - North Carolina, Registered Nurse (RN) Single State - Nursing License - Virginia Department of Health Professionals (VADHP), Registered Nurse License (RN) - Nursing License - Compact/Multi-State License
