Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for adults with severe mental illness (SMI) who are most at-risk of psychiatric crisis, hospitalization and involvement with the criminal justice system. ACT is one of the oldest and most widely researched evidence-based practices in behavioral healthcare for people with severe mental illness.
ACT uses a multidisciplinary team approach with assertive outreach in the community. The team is comprised of a Supervisor, Team Lead, SUD Clinician, Vocational Clinician, Case Managers, Nurses, Peer Support Specialist and
Administrative Assistant. The consistent, caring, person-centered relationships have a positive effect on outcomes and quality of life for the individuals that we serve. Research shows that ACT reduces hospitalization, increases housing stability, and improves quality of life for people with the most severe symptoms of mental illness.
RBHA is currently looking for a qualified individual to join our multidisciplinaryteam. This individual will need to have a passion for community based services, commitment to help adults with SMI make progress on their goals, enjoy a fast
paced environment, be a strong team player and be eager to learn and grow from this multifaceted and enriching service delivery model.
Essential Functions
Provide community based services to include assessment, therapy, casemanagement, independent living skills, care coordination and crisis response.
Implement evidenced based treatment interventions to include CBT, CBTp and Motivational Interviewing to promote recovery.
Responsible for the development and implementation of Individual, Family, and
Group treatment interventions.
Link individuals to community resources to help meet basic needs.
Provide a minimum of 75% of services in the community.
Attend daily team meetings and provide updates on any interventions or individual needs within the
last 24 hours.
Effectively collaborate with colleagues to assure client care is occurring as specified by the Assertive Community Treatment (ACT) model and the individual service plan.
Conduct diagnostic assessments and service authorization requests.
Complete individual service plans, progress notes, quarterly reports, annualdocuments and discharge plans.
Evaluate the quality of service provision and changes in individuals' condition.
Evaluate individuals' environment for safety, security and negative factors.
Maintain certification as a prescreener with DBHDS. Complete prescreens for involuntary hospitalization as needed to ensure safety for the individual and others.
Assist with crisis coordination and related clinical support, to include coverage of on call phone line.
Maintain certification for medication administration. Deliver medication to individuals and monitor medication adherence.
Assist the team with covering late nights, weekends and holidays, see below for details:

  • One assigned late night shift per week that starts at 11 am and ends at 7
  • pm.
  • Work every 5th weekend and the following week you get to choose any two
  • days off that you would like.
  • Cover on call from 5 pm-7 am every 10 weeks (No face to face community
  • work is required during overnight on call shifts.)
  • Cover 3 holidays per fiscal year. Assist with shadowing and training new employees as assigned.
    Maintain compliance with all regulatory boards, RBHA Policies and Procedures,Employee Handbook and ACT model.
    Report clinical concerns, as outlined in policy, in a timely manner to yourSupervisor/ Manager/ Director and assist in disseminating information to and from the clinical team.
    Represent RBHA in positive and professional manner.

    Position Requirements

    Education and Experience
    Graduation from an accredited college/university with a Master's Degree in Social Work, Counseling, Psychology, or closely related field required.
    License-Eligible Required
    Experience in a behavioral healthcare setting, children's mental health and/or
    emergency services preferred; Basic knowledge of ES, MH, ID/DD, and SUD treatment services, as well as the
    CSB/BHA system and structure.
    Qualifying staff will be eligible for a 4K sign-on bonus as defined by the terms and conditions of the approved sign-on bonus policy.
    Special Requirements
    Valid driver's license in the Commonwealth of Virginia.
    Preferred Credentials
    Master's degree in counseling or social work.
    LMHP-E
    Certified Precreener.
